New clients must register & book their first appointment through the office in order to activate an online account.
New and/or inactive clients are invited to download and fill out an intake form prior to their first appointment. You are free to complete only the sections you are comfortable with.

If you plan to receive full or partial coverage through an insurance provider please check with them first, to ensure the therapist you are seeing meets the criteria outlined in your policy. Each therapist holds a specific credential. At PPC, we have Registered Psychologists, Registered Social Workers, and Certified Canadian Counsellors. Your insurance provider may require that you receive a doctor’s referral prior to your session. PPC does not require this referral, but you may need one to submit a successful claim.
